Are there any privacy respecting services where you could create something similar to what you can do in Google Forms?
I’ve been looking for this solution for a long time. There are several solutions that are expensive. The free or cheap ones have much less functionality than Google. Some options to look at below. I can’t vouch for how private they are because I forget now. 
Been a minute since I researched this. But I recall that I was considering privacy when I picked these, I just can’t recall the specifics - and they may have changed since then.
